Klinischer Kommentar

Mentale Retardierung/intellektuelle Behinderung (ID) beruht entweder auf angeborenen Einschränkungen oder auf späteren Entwicklungsdefiziten vor dem 18. Lebensjahr. ID stellt eine heterogene Gruppe von Störungen dar, die durch erheblich beeinträchtigte intellektuelle Funktionen und Defizite bei adaptiven Verhaltensweisen gekennzeichnet sind. Sie betrifft bis zu 1-3% der Bevölkerung, doch die Prävalenzen variieren in verschiedenen Populationen. Die syndromale ID-Diagnose umfasst die identifizierten klinischen Symptome wie z.B. Sprachverzögerung, Hypotonie und Krampfanfälle. ID kann durch eine Vielzahl von umweltbedingten und genetischen Ursachen verursacht werden, die häufig miteinander kombiniert sind. Obwohl die ätiologischen ID-Faktoren sehr breit gefächert sind, bleibt bei der Hälfte der Betroffenen die ID-Ursache unbekannt, wobei die wichtigsten Ursachen genetisch bedingt sind. Bisher ging man davon aus, dass der genetische ID-Hintergrund hauptsächlich autosomal-dominant oder X-chromosomal vererbt bzw. durch Neu-Mutationen ausgelöst wird. Bei Verwandtenehen wird autosomal-rezessive ID häufiger vorkommen. Neben chromosomalen Aneuploidien und Kopienzahl-Variationen bedingen expandierte Tandem-Wiederholungen in bestimmten Genen ID. Wahrscheinlich können genetische Veränderungen in etwa 2000 menschlichen Genen mendelnde Krankheitsbilder mit ID als einer Komponente hervorrufen. Erste diagnostische Schritte sollten klassisches und molekulares Karyotypisieren sowie den Ausschluss von FMR1-Repeat-Expansionen umfassen, gefolgt von der Analyse eingegrenzter Panels entsprechend der begleitenden Symptomatik. Schließlich wird ein großes Panel mit Genen mit hoher diagnostischer Evidenz für die Krankheitsverursachung die diagnostische Ausbeute auf maximal 50% bei den genetischen Fällen ausweiten. Ein negatives molekulargenetisches Ergebnis schließt die klinische Diagnose jedoch keineswegs aus.
