Klinischer Kommentar

Hornhaut-Dystrophien (CDs) sind genetisch heterogene Erkrankungen, die durch die allmähliche Ansammlung von Ablagerungen in verschiedenen Schichten gekennzeichnet sind und zu Veränderungen der Transparenz und des Brechungsindex der Cornea führen. Klinisch werden diese Krankheiten in anatomische Kategorien eingeteilt, je nachdem, welche Hornhaut-Schicht betroffen ist. CDs werden demnach in 4 Kategorien eingeteilt: epithelial/subepithelial, epithelial-stromal, stromal und endothelial. Patienten mit CD können asymptomatisch sein, sofern aber Symptome auftreten, kommt es typischerweise zum beidseitigen Sehschärfe-Verlust, meist in Form eines irregulären Astigmatismus. Die Symptome können je nach Diagnose in jedem Alter auftreten. Je nachdem, welche Hornhaut-Schicht betroffen ist, weisen die Patienten auch Photophobie, trockene Augen, Hornhautödeme und wiederkehrende Hornhauterosionen auf, insbesondere bei den CDs auf Epithel-Basis, was erhebliche Schmerzen verursacht. Defekte in der Endothelzellschicht führen zu Narbenbildung, Vaskularisierung und folglich zum Verlust des Sehvermögens. Für die Mehrzahl der CDs ist eine Korrelation zwischen Phänotyp und Genotyp nicht nachgewiesen. CDs werden autosomal dominant und rezessiv oder X.-chromosomal vererbt. Da die diagnostische Ausbeute durch Molekulargenetik begrenzt ist (~70 %), schließt ein negatives DNA-Testergebnis die ophthalmologische Diagnose nicht aus.
