Klinischer Kommentar

Patienten mit progressiver Myoclonus, Unverricht-Lundborg, Epilepsie (EPM1) zeigen erste Symptome mit 6-15 Jahren mit zunehmend häufigeren Myoklonusepisoden, die z.B. das Gehen im Verlauf zunehmend behindern. In der weiteren Progression können sich ggf. Ataxie, Dysarthrie, Intentionstremor, Depression und intellektueller Abbau entwickeln. Abhängig vom Schweregrad der Erkrankung und dem Ansprechen auf die Behandlung kann die Lebenserwartung normal sein. Die EPM1-Diagnose wird häufig (90%) durch biallelisch verlängerte CCC CGC CCC GCG (Dodekamer-) Expansionen im CSTB Gen oder Compound-Heterozygotie für eine CSTB-Dodecamer-Expansion und eine CSTB-Mutation gestellt (10%). Normal sind 2-3 Dodekamer-Wiederholungen, unbestimmte diagnostische Bedeutung weisen Allele mit 12-17 Dodecamer-Wiederholungen auf (instabil, aber nicht klinisch gesichert). Pathogene ≥30 Dodecamer-Wiederholungen zeigen volle Penetranz. Die diagnostische Ausbeute ist bisher nur selten und unzureichend mit <70% beschrieben. Unauffällige Befunde schließen die klinische Diagnose keinesfalls aus.
