Klinischer Kommentar

Deoxyguanosinkinase-Mangel ist eine Erbkrankheit, die in 2 Formen zu Lebererkrankungen und neurologischen Problemen führen kann. Die Mehrheit der Betroffenen hat die schwerere Form, die wegen der schwerwiegenden Probleme in Leber und Gehirn als hepatozerebral bezeichnet wird. Bei Neugeborenen mit der hepatozerebralen Form des Desoxyguanosinkinasemangels kann es in den ersten Tagen nach der Geburt zu einer Laktatazidose mit Schwäche, verminderter Aktivität und Hypoglykämie als Folge der Leberfunktionsstörung kommen, die zum Leberversagen führen kann. Außerdem entwickeln sie fortschreitende neurologische Probleme wie schwere Hypotonie, Nystagmus und Entwicklungsrückschritte. Kinder mit dieser Form der Störung überleben in der Regel nicht länger als 2 Jahre. Bei manchen Betroffenen tritt eine mildere Form auf, bei der die Leberaffektion die Hauptsymptomatik darstellt. Diese Patienten können auch Nierenprobleme und eine leichte Hypotonie als einzige neurologische Auswirkung entwickeln. Es wurden ~100 betroffene Personen identifiziert; 15-20 % aller Fälle von mtDNA-Depletion werden durch DGUOK-Mutationen verursacht. Verminderte Aktivität der Desoxyguanosinkinase führt zu Problemen bei der Produktion und Erhaltung der mitochondrialen DNA. Desoxyguanosinkinase-Mangel wird autosomal rezessiv vererbt. Das Panel umfasst 21 Leitlinien-kuratierte Gene. Die DNA-Diagnose-Ausbeute ist nicht bekannt, und ein negatives molekulargenetisches Testergebnis schließt die klinische Diagnose nicht aus.
