Klinischer Kommentar

Skelettdysplasien sind eine äußerst vielfältige und komplexe Gruppe jeweils seltener Krankheiten, die die Entwicklung und Homöostase des Skeletts beeinträchtigen. Spondyloepimetaphyseale Dysplasie geht einher mit extremem Kleinwuchs (66-71 cm) mit kurzem Hals und tonnenförmiger Brust, Brachydaktylie, Makrozephalie und schwerer Mittelgesichtshypoplasie. Zu den Besonderheiten der Wirbelsäule gehörten Platyspondylie mit multiplen Halswirbelspalten. Heterozygote Träger werden nur etwa 150 cm groß, entsprechend einem milden, proportionierten Kleinwuchs-Phänotyp. Spondyloepimetaphyseale Dysplasie im engeren Sinn wird autosomal rezessiv vererbt, wiewohl andere Mutationen in demselben Gen sich bei allelischen Leiden dominant auswirken – ebenso wie Mutationen in anderen Genen, die differentialdiagnostisch mitunter schwer abzutrennen sind. Die DNA-diagnostische Ausbeute ist nicht genauer bekannt. Daher bedeutet ein unauffälliger genetischer Befund keinen Ausschluss der klinischen Verdachtsdiagnose, insbesondere da die Differentialdiadiagnose mit (klinisch eng) verwandten Skelettdysplasien sehr schwierig sein kann.
