Klinischer Kommentar

Das Bartter-Syndrom umfasst eine Gruppe von Tubulopathien mit Normo- oder Hyperkalzurie, Hypokaliämie und Normo-Magnesiämie, die sich in einigen Fällen bereits vor der Geburt mit Polyhydramnion bemerkbar machen. Im Säuglingsalter kommt es bei den Betroffenen häufig zu Gedeihstörungen mit Dehydratation, Obstipation, Polyurie, Osteopenie und Nephrokalzinose. Klinisch unterscheidet man zwei Hauptformen des Bartter-Syndroms bzgl. des Symptombeginns und des Schweregrads. Die Prävalenz liegt weltweit bei etwa 1/1000000. Das Syndrom kann durch Mutationen in mindestens fünf Genen verursacht werden. Mutationen im SLC12A1-Gen verursachen Typ I; Typ II entsteht durch Mutationen im KCNJ1- und Typ III im CLCNKB-Gen. Typ IV kann durch Mutationen im BSND-Gen oder durch eine Kombination von Mutationen in den CLCNKA- und CLCNKB-Genen entstehen. Demnach kommt neben dem häufig vertretenen autosomal rezessiven Erbgang auch ein digenisches Muster vor. Bei einigen Patienten mit Bartter-Syndrom bleibt die genetische Ursache der Störung unbekannt (obwohl auch zusätzliche Gene differentialdiagnostisch miteingeschlossen werden), sodass ein negatives molekulargenetisches Ergebnis die klinische Diagnose nicht ausschließt.
